After you purchase your item, we will email you a form to fill out and confirm placement of your tooth cap.
Price includes impression mold kit which will be mailed to you. This mold kit includes: 1 tray, 2 Mold Putties, Gloves and a box to send the mold back to us. Don't worry there will be instructions!
